Thank you for choosing NeuroCare Outpatient Rehabilitation to provide the care and comfort for you, your family and caregiver.  We are confident that you will recognize that your quality of life will be the most essential part of our care as we meet your physical, emotional and daily needs.

NeuroCare Outpatient Rehabilitation is committed to providing quality rehabilitation services for our patients.  We take a team approach in the delivery of care.  We believe that physicians, nurses, therapists, social workers, and family are the key to the continuum of care provided by our program that ultimately enhances the overall quality of life for each of our patients.  All care is under the direction of the patient’s personal physician and the outpatient rehabilitation medical director.  We will do our best to answer any questions you may have concerning your care and treatment and the changes that you will experience during this time of your life.

NeuroCare Outpatient Rehabilitation accepts most forms of insurance and is an in-network provider for most insurance plans.

Expectations of Patients, Families and Caregivers

The most important members of your rehabilitation team are you and your caregivers. We encourage participation in setting goals, program planning and therapy activities, as well as in education and training activities that are critical to your overall success.   We believe in planning for discharge from day one and setting team goals that help you reach your maximum potential.  Active involvement in conferences, groups and educational classes allow you to return to recover, restore and renew your  role as a productive member of the community.

Your Valuable Feedback

It is our policy to actively seek feedback from persons served to ensure that NeuroCare Outpatient Rehabilitation helps persons with disabilities to regain their lives with dignity, independence and productivity. We encourage feedback during the recovery process by regular team conferences. We use your feedback to recognize employees for their good work and to improve our services. You will be asked to complete a satisfaction survey prior to discharge. You may also receive a call from a third-party patient satisfaction company. We appreciate the time you take to give your open and honest feedback.
